Your First Appointment Information

At Treichler Sports & Family Chiropractic- Physical Therapy, your visit is geared toward the detection of the cause of your condition. It is our goal not to focus on your pain, but the cause of your pain. This begins by having a thorough consultation with the doctor. During the consultation we will learn about your past medical history and your present condition. This is followed by a physical examination that will be tailored to your problem.

Our doctors take the time to listen to you. Please leave adequate time (approximately 45 to 60 minutes) for your first appointment.

We urge you to write down your questions prior to your visit, so that no question goes unanswered.

Following your initial examine we will discuss your “Care Plan” and go over any special instructions you need to in order to obtain the
fastest results. We like to begin treatment immediately or as soon as we can- based on your symptoms.

What treatment should I expect?

Subsequent visits typically occur two to three times per week, depending on the patient’s needs. These sessions can include exercises, stretches, hands-on care by provider, various modalities to help decrease pain and/or inflammation and an educational component.

As you progress, the exercises will also be progressed to include specific sport and/or work-related simulation exercises if necessary. A re-evaluation will be completed approximately every fourteen to thirty days to assess your progress. Each re-evaluation will be faxed to your physician to keep him or her updated. When all goals are sufficiently met, as decided by you and your provider, you will be discharged to an independent home exercise program.

The primary goal of care is to get the patient active in a safe way. Improving their lifestyle and health!

What to Bring to Your First Appointment:

  1. A list of current prescription and over-the-counter medications with the dose and frequency.
  2. Your insurance card(s).
  3. A valid photo ID.
  4. Physician referral forms (if required by your insurance carrier)
  5. Pertinent information about your medical and surgical history
  6. Police or accident report if applicable
